Is there always a fifth Monday?

No. A month has five of a given weekday only when it is long enough and starts in the right place — roughly four or five months a year for any particular weekday. The tool says when the occurrence does not exist rather than silently sliding to the next month.

Which holidays are defined this way?

Most American ones: Martin Luther King Jr. Day is the third Monday in January, Thanksgiving the fourth Thursday in November, Memorial Day the last Monday in May. UK bank holidays use the same shape.
