Which countries use a 24-hour clock?

Most of the world in writing, including all of continental Europe, Latin America and much of Asia. The 12-hour clock dominates in speech in the US, UK, Canada, Australia and India — which is why interfaces should follow the locale rather than the developer.

Why does my time show '午後4:00'?

That is Japanese for 4 PM — the day period comes before the time rather than after it. Locale formatting handles ordering like this; string concatenation does not, which is why building a time from parts by hand breaks in about a third of locales.
